First, we’ll sit down with you to discuss what you might be entitled to under a Support at Home Package. We’ll help you understand the potential benefits and services you could receive, making sure you’re aware of all your options.

Next, we’ll introduce you to My Aged Care, the gateway to accessing government-funded home care services. We’ll explain how the system works, walk you through the application process, and provide helpful tips to ensure your application is as strong as possible.

We’ll guide you through filling out the forms, making sure you understand each part of the process. And remember, this is all done free of charge, with no obligation to choose Live Easy as your provider.

Once your application is submitted, we’ll keep an eye on things for you. We’ll monitor My Aged Care and let you know when your funds become available. This way, you’re always in the loop, and again, this comes at no cost and with no obligation to appoint us as your provider. Apply for an assessment

To apply for an assessment to determine if you are eligible for services, and if eligible, which type of services you can receive.

Book a face-to-face appointment with an Aged Care Specialist Officer (ACSO): 1800 277 475

After you applied for an assessment, a referral will be generated to a local assessment organisation.

Wait for a call from a local assessment organisation

A delegate from an assessment organisation will call you within 2 weeks of accepting your referral to ask you a few questions and book you in for an assessment.

Have your assessment at your own home, in person

An assessor will come to your house and ask you some questions about your current situation and needs so they can recommend the support and services that might suit you.

Receive an assessment result

After your assessment you will receive your Notice of Decision letter, which is a letter about the outcome of your assessment, and a copy of your support plan.

The letter contains:

information about the services you’re approved to receive

approval for any short-term support, if applicable

the reasons and evidence supporting the assessor’s decision

your rights to have the decision reviewed if you disagree with it

a copy of your support plan.

This will be provided shortly after your assessment by post or email. You can also view your support plan by logging into your My Aged Care Online Account.

Wait for funding to be assigned

You will need to wait for funding for Support at Home to be assigned before you can start receiving services.

The wait times for funding depend on the Support at Home supports you are approved for.

Interim funding:

When wait times for ongoing funding in the Support at Home Priority System are longer than expected, you may be assigned interim funding.

If this happens, 60% of your approved funding will be allocated, allowing you to receive the most critical services to help you remain living at home. The rest of your budget is assigned as soon as funding is available.

Receive a letter when a package is assigned

Once your funding is assigned, you will receive a letter telling you that you’ve been allocated Support at Home funding, including information about any short-term pathway approvals.

This letter includes your referral code (your chosen provider will need this code to see your assessment information, support plan and package level).

You may need to complete an Income and Means Assessment to determine your contribution percentage. This is determined by an income and assets assessment.

Note: You can get your income assessed at any time, even before you start services, however, the outcome is only valid for 120 days. If you don’t start your package during this time, you will need to complete another assessment.

Also Note: If you don’t provide your income and assets information to Services Australia, you will be given a ‘means not disclosed’ status and your contributions will be set at the maximum rate.

Choose a provider and enter an agreement

Choose a home care provider that best suits your needs

Enter into a Home Care Agreement with your chosen provider within 56 days from the date of your letter. If you need more time, call my Aged Care to ask for 28 days extension. Otherwise your Support at Home assigned funding will expire.

Purpose:
Long-term support for people needing ongoing services at home
How It Works:
Assigned 1 of 8 classification levels.
Budget:
Quarterly funding (every 3 months).
Flexibility:
Unspent funds carry over (up to $1,000 or 10%)
IMPORTANT:
Funding cannot be used for equipment or home modifications
→ Must be funded through AT-HM pathway

Short-Term

CARE PATHWAY

Short-Term

CARE PATHWAY

Purpose:
Help to maintain or regain function via intensive allied health and/or nursing services.
Duration:
Up to 16 weeks
Funding:
Up to $6,000 per episode
Focus:
Intensive allied health (e.g. physio) + nursing support

Palliative

END-OF-LIFE PATHWAY

Palliative

END-OF-LIFE PATHWAY

Purpose:
Support for people with ≤ 3 months life expectancy
Duration:
12 weeks (extendable to 16 weeks)
Funding:
One-off $25,000
Focus:
Personal care + domestic support (alongside palliative care services).

Purpose:
Separate funding for equipment and home modifications.
Funding Tiers:
Low: $500
Medium: $2,000
High: $15,000+
Timeline:
12 months to use funding
IMPORTANT:
This funding isn’t just for buying equipment/home mod itself - it also covers:
• Assessment (e.g. OT)
• Coordination/admin
• Equipment or home modifications
